Required Documents for Invitation letter
The patients who want to receive treatment in Thailand have to contact https://13.250.98.247/wp-content/uploads/2021/04/couple-jogging-park1-1.jpg and provide necessary documents which includes:
- Medical certificate/ Medical report/ “Fit to Fly” Health certificate
- A copy of passport (with a validity not less than 6 months) (Both patient and companions)
- Proof of relationship documents translated in English (House registration, Marriage certificate, Birth certificate, etc.)
Once you have provided us with all necessary details and documents, we will provide and arrange the following to assist you accordingly within 48 hours:
- Invitation letter
- Pre-appointment schedule
- Travel advice and Travel warning
- Van transfer service
- Other Convenience services
Required Documents for Visa Application and Certificate of Entry
In the visa application process, the patient need to prepare these required documents below:
- Visa application form (Filled out) – (Both patient and companions)
- A request letter for permission to enter Thailand for medical treatment
- A consent to comply with Thailand’s disease control measures, including 14-days quarantine at the hospital
- Your intended plan to stay in Thailand
- Recent photograph of the applicant(s) (4x6cm.)
- Passport (with a validity not less than 6 months) – (Both patient and companions)
- Invitation letter from the health care provider
- Medical certificate/ Medical report/ “Fit to Fly” Health certificate
- COVID-19 Test result (Negative or Undetected)
- Medical Travel Insurance (value 100,000USD/Person) – (Both patient and companions)
- Financial documents (Such as Bank statement, Financial Guarantee Letter, etc.)
- Affidavit of Support Letter (Not exceeding 3 persons)
- Proof of Relationship documents translated in English(House registration, Marriage certificate, Birth certificate, etc.)
- A copy of flight details such as Flight Number and Date & Time of flight (Both patient and companions)
Note: Please note that the consular officers reserve the rights to request for additional documents when they deemed it necessary.
Required Documents for Confirmation Letter
Once the Visa and Certificate of Entry were issued, the patient shall submit the following documents for a Confirmation Letter for Alternative Hospital Quarantine and the Hospital Appointment Documents to https://13.250.98.247/wp-content/uploads/2021/04/couple-jogging-park1-1.jpg within 72 hours before departure.
- Medical certificate/ Medical report/ “Fit to Fly” Health certificate
- COVID-19 Test result (Negative or Undetected) (issued within 72 hours before summit the documents)
- Medical Travel Insurance (value 100,000USD/Person) – (Both patient and companions)
- Affidavit of Support Letter (Not exceeding 3 persons)
- Proof of Relationship documents translated in English (House registration, Marriage certificate, Birth certificate, etc.)
- A copy of passportand Visa (Both patient and companions)
- Certificate of Entry
- A copy of flight details such as Flight Number and Date & Time of flight (Both patient and companions)
Required Documents upon Arrival in Thailand
Upon arrival in Thailand, the patients and companions are required to pass the disease screening point and present the required documents below,
- Certificate of Entry (COE)
- “Fit to Fly” Health certificate (issued within 72 hours before departure)
- Medical certificate with COVID-19 Test result (Negative or Undetected) (issued within 72 hours before departure)
- Medical travel insurance (value 100,000USD/Person)
- Financial documents (Such as Bank statement, Financial Guarantee Letter, etc.)
- Confirmation Letter for Alternative Hospital Quarantine
- T.8 Form(http://www.thaiembassy.org/london/contents/filemanager/document/T8_form.pdf)
The patients can contact us at Airport Booth at gate number 10 (Arrival Hall). Before heading to the hospital, we will check everybody’s temperature (Initial assessment), if it’s not exceeding 37.5 Degree Celsius, we will provide the patients a van transfer service to the hospital (Free of Charge).
Upon arrival at the hospital, all patients and companions have to undergo the COVID-19 Test at the hospital.If it shows Negative or Undetected result, the patients will receive treatment and have to quarantine in the hospital for at least 14 days as well as the companions. Our well-trained team will assist all patients and take care of your health and needs.
After quarantine at the hospital, we will provide the following documents:
- Quarantine certificate
- Personal Health Record / Discharge Summary
